​​​​​​​​​​​​​​​​​​​​​​​​​​​​​Access your or your minor's official immunization record through the secure Kentucky Immunization Registry (KYIR) Public Portal, a free service of the Kentucky Department for Public Health that allows individuals, parents and legal guardians to access official immunization records from KYIR.

The portal enables residents to access, save and share their vaccination records from KYIR, a web-based system health care providers use to report vaccines they administer.

Portal users may choose to receive a .pdf copy of their complete records, school certificates for students younger than 18, just their COVID-19 vaccine record as a .pdf file or SMART Health Card, a digital copy of your COVID-19 vaccine record that can be stored on your phone as a QR code.

Visit KYIR Public Portal and follow the steps below to access your immunization record. You will be asked to enter information about yourself or your child to view and download/print records.

Step-by-Step Instructions

  1. Go to KYIR Public Portal (you must be using Google Chrome, Microsoft Edge, or Mozilla Firefox).
  2. Enter your or your child's name, date of birth (MM/DD/YYYY) and gender.
  3. Select your relationship to the patient whose record you are accessing. Note: Parents cannot search and access their adult (18+) child's record.
  4. Indicate how you would like to receive a code to access the immunization record (either mobile phone or email), then enter the phone number or email you or the patient used when registering for the vaccination.
  5. Click Search to locate the record. Note: A search result will only appear if there is an exact match in KYIR of the name, date of birth and cell phone or email used at registration for the vaccine.
  6. To view, save or print your full immunization record, click Download Full Record on the top right corner of the screen.

To view, save or print your COVID-19 Record and QR code, click Download COVID-19 Record.

To view your COVID-19 QR code, click View COVID-19 QR Code. If you are viewing this on an iPhone with iOS 15.1 or later, you will see a button to add the record to your Apple Health app and Apple Wallet app.

Note: If you are downloading your COVID-19 Record on a mobile phone, the record with your QR code will appear in a new browser tab. If you are downloading your COVID-19 Record on a desktop, the record with your QR code will appear as a PDF.

  • Some Internet browser configurations may cause PDFs generated in KYIR Public Portal to download and not display immediately. If your immunization record does not display, check the download folder on your device for the downloaded file. Chrome and Firefox are recommended when using KYIR Public Portal.
  • It is important that the immunization record you download from KYIR Public Portal is saved securely on your personal/private device and if you print your immunization record, please store it in a safe place.

​​​​​​Full Immunization Record Your full immunization record lists all reported vaccines you or your child received in Kentucky,  reported immunization dates and age at the time of immunization. 



Official COVID-19 Record You have the option to receive a record with only your COVID-19 vaccine history. This record does not include history of other immunizations listed on your full immunization record.

Official Immunization School Certificate A certificate that meets the state mandated criteria for vaccination and demographic information recorded in 903 KAR 2:060.

SMART Health Card/QR Code The SMART Health Card is a secure, digital version of your COVID-19 immunization record. It uses a QR code on your device to verify proof of vaccination and keeps your other personal and health information private. Use it at restaurants, concert venues and other public gathering venues.



​SMART Health Cards are digital vaccination records of your COVID-19 vaccination history with a QR code from a trusted issuer such as an immunization registry, healthcare provider or pharmacy. It can be printed on paper or saved digitally. You can use a smartphone to scan it and securely save it to your smartphone or an app or simply save or print it for future use.

The QR code can be scanned by a compatible SMART Health Card verifier app at places like airports or large venues that require proof of vaccination. You also will be asked for proof of identification, such as photo ID, to ensure the privacy and security of your vaccination record.

You may have a SMART Health Card from your healthcare provider patient portal such as MyChart,  or from your retail pharmacy portal. The SMART Health Card is your consolidated COVID-19 vaccination record in KYIR available though the KYIR Public Portal.

The web-based, statewide  Kentucky Immunization Registry (KYIR) collects and reports immunization data across a variety of metrics.

The system protects data quality and combines individual immunization information into a single accurate record. KYIR can receive and send data securely to ensure privacy, confidentiality and accuracy.​

Note: The information in your KYIR record is collected from your healthcare provider. Enter the information you gave your healthcare provider to improve your match rate.



​The KYIR Public Portal will only allow you to search your own or your minor child’s record. Security and protection of patient records is extremely important. The public portal will not allow you to access another person’s record. Here’s how it works:

  • The KYIR Public Portal uses “two-factor authentication” to verify your identify. After entering your information, you select either text or email to receive your verification code.
  • You will receive a verification code to the phone (by text) or email address on record IF the information you entered in the portal matches exactly what was reported to KYIR (first name, last name, date of birth, gender, and email or phone). Once you enter the verification code in the portal, you can access your record.
  • ​If the KYIR Public Portal does not find your record, you can complete this online form (redcap link) with proof of your identification attached to request an update to access your immunization record, or you can contact the KYIR Public Portal Helpdesk for assistance, Monday – Friday, 8:00 AM - 4:00 PM (Phone: 502-564-0038).
If you access your COVID-19 Vaccination Record which has a QR code, you can use this QR code to securely and conveniently provide your proof of vaccination. You will also be asked to provide a photo identification when you show your QR code to ensure security of your vaccination record.


  • Print the image or PDF.
  • Store it securely on your smartphone or electronic device:
    • Download and save the PDF to your documents
    • Take a screenshot and save it to your photos. Tip: Mark the photo as a favorite so you can easily find it.
  • Save it  securely on your smartphone or electronic device through an app such as: Apple Wallet and Health  or Common Pass. Please see the tips below for Apple and Android devices.

Troubleshooting

  • Ensure your electronic device camera is set up to scan QR codes (see your device/camera settings)
  • Allow your application to access your device camera (see device settings > privacy > camera and enable access for the app)

Apple devices

Visit Apple Support for information/support (add verifiable COVID-19 vaccination information to Apple Wallet and Health)

iOS 15.1 and higher (visit Apple Support to find your OS version)

  1. Obtain your SMART Health Card.
  2. Scan the QR code using your device camera.
  3. A popup will appear and you can save the record by clicking Add to Wallet and Health.
  4. Your record will be saved in the Apple Health app and in your Apple Wallet.

Visit Google Support - Add your COVID-19 Card to your Android device for information/support

OS 8.0 or higher

  1. Download the CommonHealth app.
  2. Obtain your SMART Health Card .
  3. Scan the QR code in the CommonHealth app and confirm pass creation. The app will retrieve your health status and generate a CommonPass with your record that you can access through the app.
  4. Samsung devices will prompt to download your record to Samsung Pay by clicking the Add to Samsung Pay link. Open Samsung Pay and click COVID-19 Vaccine Pass.



The Kentucky Immunization Registry was established fairly recently in 2015. Therefore, historical data is limited with the most complete data dating back to 2005. Your immunizing providers may not be sending patient data, both current or historical, to our Registry. Sadly, it is not currently required for Kentucky immunizing providers to report patient data to us.  


​The SMART Health Card you have saved on your phone will not automatically update with this information. You will need to go into the KYIR Public Portal and obtain the updated QR code to update your record on your phone.

If you are unable to locate your or your minor's immunization records in the KYIR Public Portal, please complete this online form and attach proof of your identity to request an update to access your immunization record.

Now available: COVID-19 immunization records from KYIR now contain a SMART Health Card digital QR code.

The COVID-19 card you received at the time of vaccination issued by the CDC cannot be replaced with another card.

To have your KYIR immunization record faxed to you, please complete the KYIR Release of Immunization Record form and fax to our helpdesk at (502) 564-4760.

Please be advised risks are associated with sending identifiable information by email. If you submit a record release form by email, encrypting the email before sending is recommended to ensure information privacy and security.
