Transition Resources

Helping young people move from:

  • School to work
  • Pediatric to adult health care
  • Living at home to independent living

We work closely with our young people and their families to help them plan for the future.

Resource Rooms
Transition areas are located in some of the commission's clinics. These resource rooms have computers with Internet access, books and software about preparing for college, self-care and related issues. While visiting the clinic, you can use the computer to log onto parent bulletin boards and search other support materials.
