Transition Resources

Helping young people move from:

  • School to work,
  • Pediatric to adult health care,
  • Living at home to independent living.

We work closely with our young people and their families to help them plan for the future. If you are a young person with a special need age 14 - 21 or the parent or guardian of of a young person, the commission would like to get your input on transition issues.

In order to determine what you need, we're conducting a survey asking about the types of transition plans teens and young adults and their families are making. You do not need to receive commission services to participate.
