Continuation Program Plan FY 13-14

Below are the FRYSC Continuation Program Plan forms to be submitted to the Division of FRYSC and your Regional Program Manager. First download the Full Continuation Program Plan Document and Timeline, which contain important information, instructions and due dates.

Dec. 21      Continuation Coversheet due to DFRYSC
Feb. 15      Complete Continuation Program Plan due

May 25 is the due date for the CPP FY13 Budget/Narrative Form. Please send this form to your Regional Program Manager for approval. After it is approved, a copy should be sent to the Frankfort office for filing.
