New FRYSC Program Plan

Use the links below to view and print the FY 09-10 New Program Plan. All eligible schools not yet receiving funding for family resource and/or youth services centers may use this application package.

Important: Please print and review the New Program Plan forms, plan instructions and reference guide before filling out the forms. These documents (links below) contain important information, instructions and program and application criteria details.

New Program Plan Forms
  1. Move your computer cursor over the link to the document you want to save.
  2. Click the right mouse button once. Choose "Save target as" and select a location (drive or folder) on your computer hard drive or network server to save your document.
    You may retain the existing file name or give the file a new name. Creating a desktop folder for all related files is a good way to keep materials together and easily accessible.
  3. Repeat the process for the next document.

Open and review your saved documents at your leisure.

Timeline for New Program Plan Applications

Feb. 15, 2008 –  Due date for all new applications
Applications must be received - not postmarked - by this date.

Feb. 18-20, 2008 – DFRYSC Technical Screening of New Applications

Feb. 22, 2008 – Notification of corrections to be made to districts

March 3, 2008– FINAL application due to the DFRYSC

March, 2008 – Application Review

April, 2008 – Notification of funding to school districts

July 1, 2008 – Funding for new centers to begin

Scoring Criteria

240-day Waiver Form
 

If your school district must request a waiver for the 240-day coordinator requirement, please download the form.  Before submitting this form to the Division of FRYSC, please consult your regional program manager.

 

Intent to Apply Forms
 

The Intent to Apply form should be completed by districts with FRYSC-eligible schools that are not yet funded. Copies of the form have been mailed to superintendents. Intent to Apply forms are due by Jan. 15, 2008.
