  NOTE - Information contained on the above drug list link has NOT BEEN CHANGED since the previous August listing which was available on this web page.  ONLY the web site location of the list has been changed.  The drug list at the above web site location has a December 6, 2004 date, however, it is identical to the August 2004 listing.  The December 6, 2004 date is only due to this being the date which First Health obtained the contract for Kentucky Medicaid Pharmacy services.
