Note: 02/24/2013: FDA is alerting health care providers and patients of a voluntary nationwide recall of ALL lots of Omontys Injection by Affymax and Takeda Pharmaceuticals. The recall is due to reports of anaphylaxis, some fatal. Omontys is used to treat anemia in adult dialysis patients. Until further notice, health care providers should stop using Omontys and return the product to Takeda. For details please visit the FDA site and the Takeda/Affymax News Release.

CPT codes approved for ancillary billing include:
- X-rays, physical, speech and occupational therapy codes may be billed using the outpatient procedure codes from the Physicians fee schedule listed above. Oxygen may be billed using the Durable Medical Equipment (DME) fee schedule listed above.
- Ancillary payment methodology is outlined in 907 KAR 1:065, section 12. The current allowable procedure codes for billing nursing facility ancillary services are provided at the following links:
