Supports for Community Living Waiver
Supports for Community Living (SCL) is a home and community-based waiver under the Kentucky Medicaid program developed as an alternative to institutional care for individuals with mental retardation or developmental disabilities. SCL allows individuals to remain in or return to the community in the least restrictive setting.

How do I obtain an application?
The first step is to complete an application form for SCL services (MAP-620). You may obtain an application by calling an SCL provider in your area or by calling (502) 564-7702 or you may visit the Department for Behavioral Health, Developmental and Intellectual Disabilities (DBHDID) Web site.
Is there a waiting list?
There is currently a waiting list for services and supports and an application will allow your name to be included on the list. Additional individuals are served in chronological order as funding becomes available.
What happens after I am selected for funding?
After you are selected for funding, you must then be determined to need the level of care provided by an ICF/MR and to be eligible for Medicaid.
Who is eligible?
Individuals with mental retardation or developmental disabilities who meet requirements for residence in an intermediate care facility for people with mental retardation (ICF/MR) and who meet other Medicaid requirements are eligible for these services.
What services are available?
Supports for Community Living offers a variety of supports and services. These are based on the individual's goals, choices and priorities as identified through an individual support plan.

CDO Information
The Department for Medicaid Services (DMS) began offering the Consumer Directed Option (CDO) as an alternative for delivery of non-medical non-residential SCL services.

Transition Funding
Approved SCL Providers are eligible to apply for transition funding reimbursement. For more information, refer to the DBHDID SCL transition reimbursement page.
Enhanced Rate Information
KY Medicaid announces implementation of enhanced reimbursement for SCL group home and staffed residence providers who accept individuals who have been de-institutionalized from an intermediate care facility for individuals with mental retardation or developmental disabilities (ICF MR DD). An individual must have been institutionalized for the entire two-year period immediately preceding transition from an ICF MR DD to a community setting.
