Cooper/Clayton Method to Stop Smoking

The Cooper/Clayton Method to Stop Smoking is a 12-week program to help smokers.  This program encourages smokers to use nicotine replacement therapy while they overcome some of the behavioral aspects of nicotine addiction.  This chart indicates currently scheduled Cooper/Clayton group sessions.  Contact your local health department to determine if additional groups are under way in your area.

Nicotine Replacement Therapy

Several very different types of nicotine replacement therapy (NRT) are available as prescriptions and over-the-counter purchases.  Review this chart to determine which type of NRT will be more suitable for your patients: NRT chart.

Kentucky's Quit Line Fax Referral Form

The Fax Referral Form is designed to allow health care providers the ease of referring patients/clients to the services of Kentucky’s Tobacco Quit Line.  By having a patient/client sign the form and provide basic contact information, the Quit Line is able to start the quit process by making a proactive contact with the referral. 

Statistics have shown that tobacco users who are encouraged to quit by their health care providers are more likely to be motivated than those who are not.  This form will allow the referring provider to receive updates and progress reports on the referral and allows a dual opportunity for cessation services to be the focus of the telephone counseling and the face-to-face visits to the health care provider.

All information is considered confidential and can only be released by the patient/client signature for consent.  For additional information about the Quit Line, please refer to this fact sheet, also available in Spanish.

 

Tobacco Quit Line poster

This is a colorful 8.5 inch by 14 inch poster available in English and en español  .  These posters can be placed in public places with high foot traffic to increase awareness of the quit line.  Please feel free to use. Other campaign materials include:

Health departments and other health care agencies that would like to distribute quit line cards may order cards from the Smoking Cessation Leadership Center's Web Site  http://smokingcessationleadership.ucsf.edu/ There is a nominal charge based on the number of cards ordered.
