Cooper/Clayton Method to Stop Smoking

The Cooper/Clayton Method to Stop Smoking is a 12-week program that encourages smokers to use nicotine replacement therapy while overcoming some of the behavioral aspects of nicotine addiction. This Cooper Clayton class list contains currently scheduled classes and contact information.  Contact your local health department to determine if additional groups are under way in your area.

Kentucky's Quitline Fax Referral Form

The Fax Referral Form allows health care providers the ease of referring patients/clients to the services of Kentucky's Tobacco Quitline. By having a patient/client sign the form and provide basic contact information, the Quitline is able to make an informed initial contact with patients interested in quitting tobacco use.

Statistics show tobacco users who are encouraged to quit by their health care providers are more motivated than those who are not. By using the fax referral form, physicians and other health care professionals can receive feedback on their referrals.

All information is confidential and only released with the consent of the patient/client. For additional information about the Quitline, please refer to this physician fact sheet.

 

Tobacco Quitline poster

This is a colorful 8.5-inch by 14-inch poster available in English and en Español. Posters can be placed in public places with high foot traffic to increase awareness of the quitline. Other campaign materials include:

Quitline Outreach Cards

Health departments and other health care agencies may order cards from the Smoking Cessation Leadership Center Website for a nominal charge based on the number of cards ordered.
