Important SIM Announcements

Our first stakeholder meeting was held Tuesday, March 17, 2015 in Frankfort. We witnessed robust stakeholder participation and established great momentum for the year ahead.

As our model design process comes to a close, on behalf of CHFS Sec. Audrey Tayse Haynes and the Kentucky SIM team, we want to thank you for dedicating your time and expertise to this process over the past 10 months. With your ongoing commitment to health system transformation and improving the health of our citizens, we believe the commonwealth is well positioned to drive forward the initiatives in this plan and truly make a positive impact in the lives of Kentuckians.

Submission of KY SIM Model Design to CMS

With your robust involvement and support for Kentucky's SIM model design process, CHFS is pleased to announce that it has submitted the Kentucky State Health System Innovation Plan to CMS as of Friday, Dec. 4, ahead of schedule. Thank you for your timely comments in the final week, resulting in the final plan for submission.

We deeply appreciate your invaluable contributions to this plan and look forward to continuing to work together towards the goal of transforming the health care system in Kentucky to a system focused on promoting value and driving population health improvement.

Appointment of KY SIM Governing Body

CHFS is pleased to announce the formal creation of the SIM governing body by administrative order from Sec. Haynes. We are excited about this team's formation and its commitment to moving SIM forward. This governing body will be known as the Kentucky Health Transformation Leadership Committee: Driving SIM and kyhealthnow Into the Future. As described in the SHSIP, the appointed members are tasked with providing detailed guidance to CHFS to help advance the SIM initiatives.

Full Draft of KY SIM Model Design

We have provided here the first full draft of the State Health System Innovation Plan for stakeholder comment. Please provide your written comments here using a special Web-based tool.

Your feedback is especially encourages on the new sections of the SHSHIP not previously presented to and reviewed with stakeholders.

Previous presented and reviewed components and the month they were released are: the Population Health Improvement Plan in May, the Value-based Health Care Delivery and Payment Methodology Transformation section in August and the Quality Measure Alignment in October.

In the interest of time, please focus your review and comments on the new sections, which are clearly organized in the Web response tool. If you lose your place and/or need to revisit the survey, you will be able to return by clicking on the link above. The response tool also contains an open-ended comments box for higher-level comments on the SHSIP as a whole.

We look forward to receiving your comments by 5 p.m. on Wednesday, Dec. 2. The Cabinet will review and incorporate stakeholder comments, as appropriate, by Friday, Dec. 4 and submit the SHSIP to CMS no later than Mon, Dec 7.

Upcoming Meetings

Two full stakeholder meetings previously scheduled for Wednesday, Nov. 18 at the Kentucky Transportation Cabinet and Thursday, Dec. 3 at the Kentucky Historical Society have been cancelled. Please ensure these meetings are removed from your calendars. We apologize for any inconvenience.

While these meetings have been cancelled and will not be rescheduled, the KY SIM Team remains committed to soliciting your feedback on the State Health System Innovation Plan (SHSIP) prior to submission to CMS. You already have reviewed several components of the plan such as the Value-Based Plan and the Quality Strategy/Metrics Plan. In lieu of in-person meetings, we instead will solicit your input on the draft SHSIP electronically.

On Tuesday, Nov. 24 you will receive a copy of the complete draft SHSIP along with an electronic form for comments on behalf of yourself/your organization to submit back to the Cabinet. We ask that this feedback be returned by COB on Tuesday, Dec. 1 to be considered for inclusion in the version to be submitted to CMS Friday, Dec. 4.

SIM Workgroup Meetings

Each workgroup has its own goals and topics, but all groups are highly interrelated and will connect and overlap in their contribution to Kentucky's final model design. See SIM workgroup charters at left.

September 2015 Schedule

No workgroups will be held in September. Rather, we will hold an all-stakeholder meeting on Sept. 22 and the SIM HIT Innovation Forum on Sept. 29. Details are provided above under Upcoming Meetings.

October 2015 Schedule

Our next HIT Workgroup Meeting will be held Friday, Oct 23, 2015 from 9 - 11:30 a.m. at The Berry Hill Mansion, Music Room (first Floor), 700 Louisville Rd, Frankfort, KY 40601.

SIM Value-Based Plan Drafts

Value-Based Health Care Delivery and Payment Methodology Plan

The SIM grant requires submission of a draft Value-Based Health Care Delivery and Payment Methodology Plan. The draft was reviewed and discussed by stakeholders at the two workgroup sessions held on Aug. 26 and 27, 2015.

After gathering stakeholder feedback, the Kentucky SIM Team updated the plan accordingly and submitted the draft plan to CMS on Sept. 15, 2015. The final draft, as submitted to CMS, and a summary of changes  highlighting key changes made to the Aug. 21 draft are included below for your review.

Quality Measurement Alignment Plan - Draft for Stakeholder Review, Oct. 22, 2015

The SIM grant requires submission of a draft quality measure alignment plan as part of the overall Kentucky model design. The draft quality plan is being published for stakeholder review and input and has not yet been submitted to CMS. This plan has been drafted based on extensive stakeholder discussion and feedback, especially from the Quality Metrics/Strategy Workgroup.

After gathering stakeholder feedback at the Oct. 27, 2015 full stakeholder meeting, the Kentucky SIM Team will update the plan accordingly and submitted the draft plan to CMS at a later date.

Population Health Improvement Plan - Draft for CMS Submission, May 29, 2015

The SIM grant requires the submission of a Population Health Improvement Plan (PHIP) that links payment and care delivery reforms with population health goals. Kentucky submitted its first draft of the PHIP May 29, 2015 and will submit the final version at the end of the design process. Extensive stakeholder input on the PHIP has already been will continue to be gathered as finalization of the PHIP proceeds in tandem with formation of the other components of Kentucky's model design.

About Kentucky's SIM Model Design Award

On Dec. 16, 2014, the Kentucky Cabinet for Health and Family Services received a $2 million State Innovation Model (SIM) design grant from the Center for Medicare and Medicaid Innovation (CMMI) of the Centers for Medicare and Medicaid Services. The SIM initiative was created for states committed to population health improvement through planning, designing, testing and supporting evaluation of new payment and service delivery models.

The objective of Kentucky’s SIM model design grant is to engage a diverse group of stakeholders, including public and commercial payers, providers, advocacy groups, employers and consumers, to develop a state health system innovation plan.
