Reporting Fraud

If you would like to report suspected fraud, waste or abuse occurring within any area of the Cabinet for Health and Family Services, call the OIG Hotline at (800) 372-2970 or complete the form below.  Your identifying data is optional, however, there maybe additional information that you could provide which will allow us to successfully investigate the allegation.  Please consider filling in that portion of the form.

Please be assured that all personal information will be kept in strict confidence.

The answers to the following questions will be helpful in our evaluation of any potential violation:

1. Who committed the fraud?
2. What exactly did the person do?
3. Where did the fraud take place?
4. When did it happen?
5. How was the fraud committed?
6. Do you know why the person committed the fraud?
7. Who else has knowledge of the potential violation?
