Adam Walsh Requirements and Forms

Pursuant to 922 KAR 1:490 all resource home applicants and household members of resource home applicants shall submit to a child abuse and neglect check for each state of residence during the past five years.

Please check the state information below for all resource home applicants and all household members of resource home applicants who have resided in a state other than Kentucky within the past five years.

States requiring their state specific form to be utilized when requesting a child abuse and neglect check (CAN check), have an X in the column titled State Requires its Form. Please click the state’s name to view and print the form and/or instructions for all states.

States which do not require a specific form to be utilized, when requesting a child abuse and neglect check, or which are not participating in the Adam Walsh and Safety Act of 2006, have an N/A in the column titled State Requires its Form. The N/A is not to be interpreted as a child abuse and neglect check from that state is not required or that the resource home applicant or household members of the resource applicant are exempt from indicating residence in ALL other states within the five-year timeframe on the DPP-157 form.

The resource home applicants and household members should complete the DPP-159, DPP-157 and other state form at the same time. All forms should be submitted simultaneously to the Records Management Section conducting the fingerprint scan, except for states indicated below that require a notary and/or original signature(s). In instances where the other state requires a notary and/or original signature(s), the requestor will need to mail the hard copy to:

Guam, Kentucky, Louisiana, Maine, Michigan, Ohio, Oregon, Rhode Island do not require a form.

Oklahoma has no central registry.

Puerto Rico has no clear child abuse registry available.
