​Help Me Grow Kentucky is a free, confidential call center for families, health providers and community service providers offering developmental screening and connections to local services and learning activities that support healthy growth for children from birth to age 5.

Developmental Screening Tools​

The first five years of your child’s life are very important. Healthy development forms a foundation for lifelong learning. Help Me Grow Kentucky provides two online screening tools to quickly check your child’s development and identify your child’s strengths as well as areas in which your child may need support.
We use the Ages and Stages Questionnaires® (ASQ-3® and ASQ:SE-2®), an evidence based, universally accepted, reliable screening tool.  It is designed to be parent/guardian completed because you are considered the expert on your child. 

Take the Screenings

Two screening tools are available. You may use either screening tool. It is beneficial to use both to get a better picture of where your child is developmentally. Each screening tool is available in English and Spanish.

Have fun completing these questionnaires with your child. Make sure your child is rested, fed and ready to play before your try the screening activities.

After you have submitted your answers, your answers will be reviewed. We will email the results to you with an explanation of recommended next steps.

Spanish ASQ:3 and ASQ:SE-2.​

The Ages and Stages Questionnaire, 3rd edition (ASQ-3™) is organized into five areas to screen for a child's abilities with communication, gross and fine motor skills, problem solving and personal-social development. Each of the five skill sets has six questions that increase in degree of difficulty. Your child may be able to perform some but not all of the items in the screening questions. Read each question carefully and answer yes, sometimes or not yet as best describes your child's abilities in each screening area.
