Division of Long-Term Services and Supports

​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​​Kentucky Medicaid's Division of Long-Term Services and Supports (LTSS) oversees programs for Kentuckians who need ongoing support due to their age, a medical condition, or a disability.  LTSS offers a variety of options to meet each individual’s unique needs. 

Programs and Services

1915(c) Home- and Community-Based Services Waiver Programs
For individuals with a disability who need nursing facility level of care and want to remain living at home or in the community

Program of All-Inclusive Care for the Elderly (PACE)
For individuals age 55 or older who need nursing facility level of care but want to continue living at home

Kentucky Transitions
For individuals who wish to move out of a nursing home or institution and into the community

Home Health
For individuals who need short-term medical care at home due to an illness or injury

Private Duty Nursing (PDN)
For individuals who need continuous skilled nursing due to a disease, illness, or injury.

For individuals who require facility-based ​​skilled nursing care or rehabilitation 

Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICF/IID) 
For individuals with intellectual disability (ID) or related conditions who facility-based care

Intermediate Care Clinic 
Provides comprehensive medical, dental, vision, and psychiatric services to individuals with intellectual disabilities or related conditions

Hospice​
Providers a range of services to individuals and their families at the end of life

LTSS News and Updates

Proposed CHILD Waiver: DMS and the Department for Behavioral Health, Developmental, and Intellectual Disabilities (DBHDID) are developing a new waiver to serve individuals aged 21 and younger. The proposed waiver will provide 1915(c) Home and Community Based Services to children, youth, and young adults with multi-system needs and complexities who meet either Hospital-Psychiatric Residential Treatment Facility or Intermediate Care Facility for Individuals with Intellectual Disabilities level of care. 

DMS collected public comments on the proposed waiver as part of the process for receiving approval from the Centers for Medicare and Medicaid Services (CMS). The public comment period ended July 15, 2025.

DMS and DBHDID will review all comments submitted and issue a formal public response before submitting the waiver application to CMS for approval. To receive email updates about Kentucky's 1915(c) HCBS waiver programs, subscribe to updates or email DMS​. ​

Slot Allocation Plan: New slots in the ABI LTC, HCB, MPW, and SCL waivers will become available July 1, 2025. Slots will be allocated gradually over several months. Review the slot allocation plan for details. 

HCB and Modell II Waiver Renewal Approved: The Centers for Medicare and Medicaid Services has approved the renewal of the HCB and Model II Waiver applications. The effective date for HCB is Aug. 1, 2025, and the effective date for Model II Waiver is Oct. 1, 2025.

As part of the renewal process, DMS held an initial formal public comment period for both waivers between March 13, 2025, and April 13, 2025 and a second public comment period from June 2, 2025, to July 1, 2025. DMS summarized and responded to all public comments after both public comment periods.

​1915(c) HCBS Waiver Rates: New rates for all six 1915(c) HCBS waivers went into effect on Jan. 1, 2025. The new rate methodology incorporates rates based on the 1915(c) HCBS rate study and the rate increases funded in the 2024-2026 state budget. ​The new rates are listed on the DMS Fee and Rate webpage.​​

Before submitting the proposed rate amendments to the Centers for Medicare and Medicaid Services for approval, DMS held a 30-day public comment period. DMS' formal response to public comment is now available for review.
