Grant Family Planning Providers
(Not Local Health Departments)
The Kentucky Department for Public Health allocates awarded federal Title X funds to local health departments, federally qualified health centers (FQHCs), FQHC look-alikes, rural health clinics and university-based primary care clinics who commit to provide family planning services in their area.
Title X core services include contraceptive services, pregnancy testing and counseling, optimal family spacing, basic infertility services, STD services, preconception health, related preventive health services (for example, cancer screening) and other preventive health services. Family planning services are provided to both females and males.

Quality Assurance Site Review
Effective July 2024 the Quality Assurance Program Review Tool has been separated into two surveys: the Family Planning Policy Review, and the Family Planning Annual Chart Review.
