Open Records Requests

​​The Cabinet for Health & Family Services is now utilizing an online platform for individuals to submit open records requests. If you would like to request records from the Cabinet, please click on the link below to submit your request. The Cabinet will then be able to process your request.

Division of Health Care

To receive records maintained by and in the custody of the Division of Health Care, please submit your request(s) in the Open Records Center or by mail, fax, or e-mail to:​

You may submit a letter identifying the records sought or complete and submit a Division of Health Care - Open Records Request Form.  Please be sure to provide your name, address and daytime telephone number in the event we need to contact you.

All requests for records must be signed by the requester.

The following information may be released by the Division of Health Care:

  • Statement of deficiencies/Plan of correction
  • Type A or B citations
  • License
  • Applications
  • Final correspondence

The Division of Health Care has five business days from the date of receipt to respond to an open records request. If the documents requested are not available for release within five days, a response will be provided informing the requester of the approximate date by which to expect the requested documents.

Note: If the records requested are maintained by and in the custody of another agency, a request must be submitted to that agency. For example, the following information is maintained by and in the custody of the Centers for Medicare and Medicaid Services:

  • Surveyor notes and worksheets regarding Medicare/Medicaid-certified health care facilities
  • Complaint narratives regarding Medicare/Medicaid-certified health care facilities

To obtain copies of surveyor notes/worksheets or complaint investigations for Medicare/Medicaid-certified health care facilities a Freedom of Information Act request should be sent to:
