Electronic Plan of Correction for Certified Nursing Homes

​The Electronic Plan of Correction (ePOC) application eliminates the need for paper-based communication between Certified Nursing Home providers and the Office of Inspector General, Division of Health Care when submitting and reviewing plans of correction.  

ePOC Launch in iQIES for Nursing Homes July 14, 2025

​On July 14, 2025, CMS will release the new Internet Quality Improvement & Evaluation System (iQIES) Electronic Plan of Correction (ePOC) system functionality to participating Nursing Homes and State Agencies. The use of the new ePOC system is mandatory to minimize CMS certification and enforcement work disruption.

Once ePOC functionality is enabled in iQIES, the current web ePOC system will be disabled for ePOC users. ​​​​As a Nursing Home provider, you will need to request the Provider ePOC Administrator role in iQIES after you sign up for HCQIS Access Roles & Profile (HARP) access. As we approach the Nursing Home (NH) iQIES release date, we will be posting to the QIES Technical Support Office (QTSO​) all supporting resource documents as well as training videos for NH ePOC participating providers.

Please Note:

  • You will not be able to request the Provider ePOC Administrator role in iQIES until July 14, 2025. Users however can sign up for a HCQIS Access Roles and Profile (HARP) account now, which is required to obtain an iQIES role. We will send out additional information when the iQIES Provider ePOC Administrator role becomes available.

  • For your Provider ePOC Administrator role request to be approved, your organization must have at least one Provider Security Official (PSO) assigned. CMS recommends that at least two PSOs be established for every CMS certified Nursing Home provider. If your organization needs to review which roles are available and their descriptions, there is an iQIES User Roles Matrix document available on the QTSO website: https://qtso.cms.gov/software/iqies/reference-manuals.

Details regarding the CMSNet and ePOC account registration can be found on the QTSO Access Forms site for Nursing Home Providers. Once providers complete the ePOC enrollment process, they can begin accessing the CMS-2567 Statement of Deficiencies (SOD) and enter their Plan of Correction (POC) via the secure website created by CMS .  

Requesting Accounts

Please use the information below to assist you with obtaining the two accounts that will be needed to access the ePOC application: 

  1. CMSNet Account
  2. ePOC Account

ePOC Individual Access

To access ePOC for only your facility, use the ePOC Individual Access site.

ePOC Multiple Facility/Corporate Access

Users needing access to plans of correction for more than one facility within the same corporation will use the ePOC Multiple Facility/Corporate Access site.

Accessing the ePOC Application

These steps may be followed at any time after registration and activation have been completed.

  1. Go to https://qtso.cms.gov
  2. Select CMSNet Information. (upper right corner of page) 
  3. Select CMSNet - Submission Access
  4. Select the appropriate state, proceed through the warning and enter your CMSNet login ID/password. 
  5. Select the web link titled CMS QIES Systems for Providers. 
  6. Select ASPEN Access (ePOC) (bottom of page) 
  7. Select ASPEN Web (upper left corner of page)
  8. Enter your QIES login ID/password to enter the ePOC application.
