Vaccines for Children Program

​​​​​​​​​​​The Vaccines for Children (VFC) program was created in August 1993 as a federally-funded program to ensure all children have access to life-saving vaccines. The VFC program is regulated by the Centers for Disease Control and Prevention and National Immunization Program. The Kentucky Immunization Branch of the Department for Public Health manages the VFC program at the state and local levels, working closely with providers to help develop and implement systems to assess and increase immunization levels statewide.

The VFC program provides vaccines at no cost to eligible children through public and private providers enrolled in the program. All vaccines recommended by the Advisory Committee on Immunization Practices are available through the VFC program. The program acts to reduce vaccine costs which create a barrier to vaccination of eligible children. It also helps keep those children in their medical home for comprehensive health care without having to refer them elsewhere for vaccinations.

Children and adolescents younger than 19 who meet one or more of the following criteria are eligible:

  • Medicaid eligible
  • Uninsured
  • American Indian or Alaska native
  • Underinsured are eligible for vaccines through federally-qualified health centers or local health departments.

How to Join the KY VFC Program

Complete the provider agreement form in the link below to begin your enrollment with the VFC program. Submit the completed form to KYVaxProvider@ky.gov or fax to 502-564-4760.
