 MAP 576 Reimbursement Information regarding the new Contract Period, July 1, 2018 – June 30, 2020

The new contract period for reimbursement of Nurse Aide Training expenses for LTC Nursing Facilities will begin July 1, 2018.

Each new Contract Period requires all Nursing Facilities to complete a new, notarized Affidavit, prior to receiving reimbursement for expenses incurred during the training of nurse aide employees.

The Map-576  and the Affidavit are located on the CHFS website: under “Nurse Aide Training”. The MAP 576 may be scanned and E-mailed with all required documentation to:

Communications, including attachments, via email over the internet are not secure. Although it is unlikely, there is a possibility that information you include in an email can be intercepted and read by other parties besides the person to whom it is addressed.  It is recommended that your Nursing Facility take steps to secure/encrypt all electronic transmissions to protect the integrity of data that contains Personal Identifying Information (PII), Personal Health Information (PHI), and/or information that falls under HIPPA guidelines.

JUNE 2018: IMPORTANT NURSE AIDE TRAINING MAP 576 REIMBURSEMENT INFORMATION

Due to an upgrade of the  Commonwealth’s accounting system, MAP 576 invoices for Nurse Aide Trainings that occurred between July 1, 2017 and June 30, 2018 MUST be received by Medicaid by Wednesday, August 1, 2018. 

Reimbursement will not be made for trainings completed prior to June 30th IF the invoice is not received by August 1, 2018. 
Map-576 forms should be submitted to: Vicki.Barber@ky.gov.
Per the contract language, billings shall be initiated upon completion of the training program, no more than thirty (30) days after completion of the training program.
