for a 1915(c) Home- and Community-Based Services (HCBS) Medicaid waiver. These waivers provide the support you need to live as independently as possible.

If you are interested in applying for 1915(c) waiver services, you first must be determined to be financially eligible for Medicaid. You can apply for Medicaid online using
kynect, by calling the
Department for Community Based Services (DCBS) at (855) 306-8959 or in-person at a
DCBS office. Documents that are helpful to have when applying are listed in the

Once you are ready to apply for 1915(c) HCBS waiver services, you can complete your application online using
kynect or in-person at an
Aging and Disability Resource Center or a
Community Mental Health Center.

Recent Updates from the 1915(c) HCBS Waiver Programs
Western Kentucky Tornado Relief: The Centers for Medicare and Medicaid Services (CMS) has approved Kentucky's Appendix K application making temporary changes to the waiver programs for participants and providers affected by the December 2021 tornado outbreak. The changes only apply to the counties listed in Appendix K.

DMS also has compiled a list of storm recovery resources for waiver participants and providers. The list includes information such as available state services, shelter, recovery assistance, health services and mental health support.
ABI Waiver Renewal Update: The application renewal process with CMS is underway for the ABI waiver. Public comment on the waiver application renewal ended Jan. 9, 2022. DMS is reviewing comments and will issue a response soon. If you have questions about the renewal process, please email DMS.
New Home Delivered Meals Option for Waiver Participants: Mom's Meals is approved to provide home-delivered meals service to Kentucky 1915(c) HCBS waiver participants. Mom's Meals delivers to participant doorsteps ready-to-heat meals that are good for up to 14 days. Review the

Kentucky's Enhanced Federal Medical Assistance Percentage (FMAP) Spending Plan Partially Approved: The
American Rescue Plan Act includes a temporary 10 percent increase in federal funding, known as FMAP, for HCBS programs. States must submit a plan to CMS before using these funds. CMS recently approved part of
Kentucky's spending plan. Review the
