Kentucky Medicaid members are issued Medicaid ID cards in the mail that look similar to the images at right. The ID card has the member’s name and a randomly generated Medicaid identification number on the front of the card.
When you receive your card, make sure your name and any dependents’ names are spelled correctly. If there are errors on your ID, contact the Department for Community Based Services toll free at (855) 306-8959.
Your ID card is valid as long as you are Medicaid eligible. Keep your card in a safe place where it easily is accessible. Do not throw away your card. If you lose your card, contact the Department for Community Based Services toll-free at (855) 306-8959 to request a replacement ID.
If you are enrolled in a managed care organization (MCO), you also will receive a card from your MCO. Watch your mail for your MCO card from;

Starting Jan. 1, 2025, Anthem will no longer be a Medicaid Managed Care Organization, or MCO, in Kentucky. This does not change a member’s Medicaid eligibility. Medicaid coverage will not be interrupted.
