All Kentucky Medicaid members receive a permanent Medicaid Identification (ID) card after enrolling in the program.
The ID card comes in the mail and includes the member's name and Medicaid identification number on the front.
Important Information about your Medicaid ID Card
Here is what you need to know about your Medicaid ID card.
- Everyone in your household who receives Kentucky Medicaid will receive their own card.
- The ten (10) digit Medicaid identification number is randomly generated. It is not your Social Security number.
- Do not throw the card away. Keep it with you at all times.
- You must show the card when you go to the doctor or pharmacy.
- Keep your card, even if your Medicaid ends. If you are approved for Medicaid again, you will use the same card.
- If your card is incorrect, lost, damaged, or stolen, visit your local DCBS office or call 855-306-8959.
If you are enrolled in a managed care organization (MCO), you will receive a card from the MCO you selected.
