Kentucky Medicaid members are issued Medicaid ID cards in the mail that look similar to the image below. The ID card has the member's name and a randomly generated Medicaid identification number on the front of the card.

When you receive your card, make sure your name and any dependents' names are spelled correctly. If there are errors on your ID, contact the Department for Community Based Services toll-free at (855) 306-8959.

Your ID card is valid as long as you are Medicaid eligible. Keep your card in a safe place where it is easily accessible. Do not throw away your card. If you lose your card, contact the Department for Community Based Services toll-free at (855) 306-8959 to request a replacement ID.
